Occupational Therapist (OT) – Home Health

We're seeking a compassionate Occupational Therapist to provide one-on-one home health services throughout Long Island, helping older adults improve function, confidence, and independence.

What You'll Do

Perform occupational therapy evaluations in patients' homes. Design individualized treatment plans based on each patient's functional goals. Address limitations affecting self-care, mobility, and everyday activities. Recommend adaptive equipment, home modifications, and safety strategies that support independent living.

Educate patients, family members, and caregivers on techniques that promote long-term success. Monitor patient progress and update treatment plans as needed. Qualifications

Current New York State Occupational Therapist license. Strong clinical reasoning and communication skills. Ability to work independently in a community-based setting. Experience with adult or geriatric patients is preferred but not required. Home health experience is welcome but not necessary.
