LVN - Full Time - Day Shift - Skilled Nursing Facility

Summary of Position:  The LVN provides basic nursing care to residents within Skilled Nursing, utilizing established policies and procedures, ensuring safe and effective care.  Services are provided based on the ages of the population served; Adult through geriatric - 65 years or older and in collaboration with the multidisciplinary care team.

Assists the RN to assess and plan care by collecting resident care data through observation and interview. At the direction of the RN and in collaboration with the multidisciplinary care team implements and evaluates care. The LVN demonstrates the skill necessary to meet the individualized needs of all residents served in Skilled Nursing.
