School Counselor/Social Worker/Case Manager

SCHOOL DISTRICT 170 CHICAGO HEIGHTS, ILLINOIS TITLE: School Counselor/Social Worker and Case Manager REPORTS TO: Principal(s), Assistant Superintendent of Special Education and Special Services, and other designated administrators TERMS OF EMPLOYMENT: In accordance with the collective bargaining agreement between the Board of Education of School District 170 and AFT Local 604 PENSION: Teachers' Retirement System (TRS) SALARY RANGE: $49,144-$55,663; Commensurate with experience.

Additional stipends of ($5500) and ($3500) JOB GOAL: To provide comprehensive counseling, social-emotional support, and case management services to students, while coordinating special education processes and serving as a liaison among school, home, and community agencies to support student achievement and well-being.

QUALIFICATIONS

Valid Illinois Professional Educator License (PEL) with School Social Worker and/or School Counselor endorsement required ? Knowledge of special education laws and regulations, including IDEA and Illinois School Code ? Demonstrated ability to work collaboratively with staff, students, families, and outside agencies ?

Strong organizational, communication, and interpersonal skills PERFORMANCE RESPONSIBILITIES: A. Student Support Services

1. Provide individual and group counseling to address students' social, emotional, and behavioral needs
2. Conduct student observations, assessments, and social developmental studies as part of evaluation processes JR/JB 051126
3. Assist in identifying student needs and developing appropriate interventions and supports
4. Support development of Functional Behavior Assessments (FBA), Behavior Intervention Plans (BIP), and transition plans
5. Provide parent education and support related to student needs B. Case Management and Special Education Coordination
6. Coordinate and facilitate all aspects of the special education process, including referrals, evaluations, eligibility, IEP meetings, and annual reviews
7. Ensure compliance with state and federal timelines, procedures, and documentation requirements
8. Schedule and organize meetings, ensuring appropriate staff participation and required documentation
9. Coordinate the collection and review of evaluation data and reports
10. Maintain accurate and confidential student records and case files
11. Serve as liaison between the school, families, and outside agencies regarding student services
12. Coordinate out-of-district placements and related processes as assigned C. Consultation and Collaboration
13. Serve as a resource to teachers, administrators, and support staff regarding student needs and interventions
14. Participate in MTSS and problem-solving teams
15. Collaborate with staff on behavior management strategies and student supports
16. Provide professional development and training as needed
17. Assist in identifying service gaps and developing additional supports D. Family and Community Engagement
18. Serve as liaison between school, home, and community agencies
19. Facilitate access to community resources and services for students and families
20. Assist with referrals to outside agencies and service providers
21. Support follow-up related to attendance concerns and crisis situations E. Compliance and Documentation
22. Develop and contribute to Individualized Education Programs (IEPs)
23. Maintain service logs, documentation, and reports in accordance with district and regulatory requirements
24. Support Medicaid billing processes as applicable
