Geri-Psych Coordinator, FT (8:00Am-4:30PM)

JOB SPECIFICATIONS: Must possess a high school diploma or GED certificate. Must be able to read, write and speak the English language in order to communicate effectively and tactfully with staff, patients and the general public. Must be able to maintain confidentiality of patient information. Initial contact with patients for admission to WRH Behavioral Health Care Units Preadmission assessment of the clinical status of patient referrals and evaluations of patient's needs: Medical Clearance, Insurance Clearance, Psychiatric Clearance.

Serves as an educational resource for patients, families, hospitals and healthcare professionals upon admission to WRH Mental Health Care Units. Liaison between WRH Emergency department and Behavioral Health Care Units. Actively assist the referring facilities in addressing requirements of mental health patients to WRH Behavioral Health Care Units.

Establish relationships with community and healthcare facilities, provides education across the continuum of WRH Behavioral Health Care Units.
