RN Case Manager (Home Health)

Allegiance Home Health of Northwest is looking for a RN Case Manager in Minden, LA. The RN Case Manager manages patient health problems and needs, develop and implement a plan of care, team schedules, and maintain medical records. Utilize team to administer and carry out Plan of Care. Advise team on health maintenance and disease prevention and provide case management. Licensing without restrictions required.
