Pharmacist

Job Summary

Performance of professional functions generally associated with the practice of pharmacy in the hosptial setting. Duties/Responsibilities: · Compounding and preparing ordered pharmaceutical preparations · Contacting and conferring physicians if there are any questions or irregularities regarding medication orders · Preparing TPN solutions and Monitoring TPN Patients as required · Mixing of irrigating solutions and intravenous additive solutions as required using aseptic technique with the laminar air flow hood or CAL, or CACI for hazardous medications · Shows competencies in aseptic technique, garbing and gowning, and didactic training in IV and HD preparation · Consulting with physician and nursing staff concerning availability, form, side effects and incompatibilities of currently available drugs · Responsible for the collection of Quality Assurance/Performance Improvement data · Interprets prescription orders and contacts provider or insurer as indicated · Performs medication counseling for patients as requested · Supervises activities and checks work of pharmacy technicians · Performs other duties as assigned Required Skills/Abilities: · Excellent verbal and written communication skills.

Education and Experience

· Bachelors Degree or Doctorate of Pharmacy, must meet state requirements for Pharmacist and registered by the Louisiana Board of Pharmacy · Minimum of one year experience in hospital Pharmacy preferred
