Responsibilities

Assess psychosocial needs of patients and families. Provide counseling, crisis intervention, and grief support. Assist with care planning, advance directives, and community referrals. Requirements

Active Maryland Master Social Worker License (LMSW). 1+ year of healthcare experience (hospice preferred). Strong communication and organizational skills. Valid driver’s license and insured vehicle.
