Adult Nurse Practitioner-- Post-Discharge In Home Care (N Ft Worth / Alliance, TX areas)

Segue Health – Faith-Based Medical Practice Segue Health, a faith-based medical practice, is seeking an experienced and compassionate Nurse Practitioner to join our team serving patients in North Ft Worth, Alliance, Keller, Haslet, and surrounding areas . This role focuses on providing in-home post-discharge care management and medical treatment for patients transitioning from acute care facilities back to their homes.

Key Responsibilities

Conduct in-home patient visits to assess clinical status and provide post-discharge medical care. Review and interpret discharge summaries and progress notes. Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care. Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions.

Qualifications

Current Texas medical license and active Medicare provider number required. Nurse Practitioner certification in good standing. Ability to travel within a 35-mile radius from home to see patients throughout N Ft Worth, Alliance, Keller, Haslet, and surrounding areas. Strong communication, clinical judgment, and documentation skills.

Compensation
