Title: Manager of Housing and Health Integration Department: Community Based Care Shift: Full-Time 8:30AM - 5:00PM Pay Grade: NBU 07 Location: 4730 N Winthrop, Chicago, IL 60604 Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable - particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses - improving health for all and the well-being of our community.

The Housing and Health Integration Supervisor provides leadership and direct supervision across two complementary program areas: Heartland Alliance Health's Community Based Care services and a HUD-funded Chronic Homeless Supportive Housing program operated in partnership with the AIDS Foundation of Chicago (AFC). This is a full-time, single-site position operating five days a week.

Across both programs, this role blends program oversight, contract compliance, health outcomes monitoring, and hands-on clinical behavioral health support in a low-barrier, trauma-informedenvironmant. This role has a strong emphasis on care coordination and linkage to care for individuals experiencing homelessness, including connections to primary care, behavioral health, and psychiatric service.

The supervisor provides direct supervision to the assigned Case Managers and Outreach Workers across both program areas.

Key Responsibilities

Staff Supervision and Team Management Provides individual supervision to assigned staff on a regular basis, including case review, goal-setting, coaching, and professional development. Facilitates team meetings and case conferences to coordinate service delivery across program caseloads. Monitors staff performance against program objectives and productivity standards, and addresses gaps in a timely and supportive manner.

Ensures completion of all required participant contacts, assessments, and reassessments within required timeframes. Ensures completion of the training required by the agency and funders. Supports onboarding of new staff, ensuring familiarity with CBC protocols, documentation workflows, and linkage to care expectations.

Program Oversight and Service Delivery Ensures a low-barrier, trauma-informed, and participant-centered environment is maintained at all times. Ensures the health team is visible and accessible to participants during program operating hours, reinforcing the program's No Wrong Door approach, including direct engagement with participants.

Oversees daily service delivery including health screenings, enabling services, benefits navigation, referrals, and linkage to care. Supports the team in maintaining accurate and timely documentation in both Athena Practice and Case. Worthy. Reviews and approves case notes, goal plans, and service documentation prior to reporting periods.

Addresses participant concerns, behavioral incidents, and service access issues as they arise, escalating to the Program Manager as needed. As a licensed clinician, provide direct individual behavioral health support to Drop-In participants, including brief interventions and crisis stabilization while external referrals are pending.

Compliance and Reporting Ensures program activities and documentation meet funder contract requirements and program standards. Monitors compliance with AFC lease submission requirements, ensuring 90% of leases are submitted without deficiency and revised leases are resubmitted within 3 business days. Ensures 95% of assigned leases are maintained without lapse throughout the contract period.

Overseas timely entry of all participant data into Case. Worthy and Athena Practice, including assessments, case notes, referrals, and lab results for people living with HIV/AIDS. Tracks housing stability outcomes including 12-month and 24-month retention benchmarks and reports findings to the Program Manager. Supports the Program Manager in preparing for AFC oversight visits, data audits, and program evaluation activities.

Ensures compliance with HIPAA and the Illinois AIDS Confidentiality Act across all program documentation and communication. Prepares and submits required program reports accurately and on time. General Responsibilities Participates in progressive discipline processes and performance improvement plans in collaboration with the Program leadership and HR.

Represents the CBC division in internal and external meetings as assigned. Completes other duties assigned by the Program Associate Director.

Qualifications

Education

and Licensure Master's degree required in Social Work, Counseling, Psychology, or a related clinical field (e.g., MSW, MA/MS in Counseling, or equivalent). Active, unrestricted independent clinical license in Illinois required (LCSW, LCPC, or equivalent). Experience: Minimum 3 years of experience in case management, care coordination, or social services.

At least 1 year in a supervisory or lead capacity. Experience working with people experiencing homelessness, chronic homelessness, and/or serious mental illness strongly preferred. Experience with HUD-funded supportive housing programs and/or HIV/AIDS services preferred. Knowledge, Skills & Abilities Strong understanding of trauma-informed care, harm reduction, and low-barrier service models.

Proficiency with electronic health records (EHR) and case management systems (Case. Worthy, Athena Practice, HMIS). Excellent clinical, communication, and interpersonal skills; able to build trust with diverse, high-need populations. Demonstrated ability to manage multiple priorities, meet deadlines, and maintain accurate documentation under pressure.

Commitment to equity, cultural humility, and person-centered care. Working Conditions Full-time, on-site position at the New Hope Drop-In Center (4730 N. Winthrop Ave, Chicago, IL). Regular evening or weekend work may be required based on program needs. Occasional travel to off-site meetings, trainings, or partner locations within Chicago.

Work environment may include exposure to individuals in crisis or experiencing acute behavioral health symptoms; ability to de-escalate and respond appropriately is essential.
