Primarily Monday-Friday schedule with only one rotating on-call weekend each month. $1,250 Hiring Bonus after 90 days! What You'll Do
Create personalized healthcare plans that truly support each individual’s needs Train and support staff so they feel confident providing care Conduct nurse visits and attend medical appointments with clients Participate in ISP & IDT meetings, ensuring each person’s voice is heard Maintain accurate medication and treatment records to keep care safe and consistent Schedule & Pay: Job: Full-Time or Part-Time | Monday thru Friday with rotating on-call weekends shifts!
Pay: $30.00 per hour DOE.

Requirements
Licensed Practical Nurse (LPN) certified in New Mexico At least 1 year of experience working with individuals with disabilities (experience with severe/profound disabilities is a big plus!) Experience with feeding tubes (jejunostomy/gastrostomy) preferred. Other Requirements: Must pass TB test and background check. Must have own reliable transportation, valid NM driver’s license, insurable driving record and proof of current liability insurance.
Must be able to sit, stand, walk, bend, stoop, carry, and perform keyboarding/typing tasks frequently. Must be able to squat, reach above shoulder(s), kneel, balance, push, pull and lift 50 lbs. occasionally. Adelante is a drug free workplace.
