We are hiring Full-Time, Part-Time, and PRN , but here's the difference: Part-Time & PRN nurses can select a focused territory. Full-Time nurses must be available to cover all of Montgomery County based on patient needs.

What You'll Do

Under the direction of the RN and physician-established plan of care, you will: Provide skilled nursing care in patients' homes Perform focused nursing assessments Monitor vital signs and changes in condition Provide wound care and ordered treatments Support medication management and patient education Reinforce disease-management education Monitor patient response to treatment Identify and report changes in condition Educate patients, families, and caregivers Communicate with the supervising RN and interdisciplinary team Follow physician orders and established plans of care Complete accurate and timely documentation using Well.

Sky What We're Looking For Active Maryland LPN License Required Graduate of an approved Practical Nursing program Reliable transportation Strong clinical and communication skills Excellent organizational and time-management abilities Ability to independently manage home health visits Full-Time candidates must be willing and able to cover all of Montgomery County Home Health Experience Preferred, but Not Required Well.
