Director of Case Management

We are seeking a Director of Case Management to join our amazing team at our beautiful and luxurious community. This highly polished professional will serve as the primary liaison between residents, their families and community team members. In this role, you will be the first line of support to residents in navigating the admission and discharge process.

This includes providing orientation, social/emotional support, arranging for needed services and maintaining case notes as per company policy and regulatory rule. (Sun-Thurs 9-5:30PM)
