You’ll work closely with residents, caregivers, program staff, physicians, and other healthcare professionals to ensure medical needs are identified, appropriately addressed, documented, and incorporated into each individual’s overall support.

Your Responsibilities

Nursing Care & Health Monitoring Assess and monitor residents’ physical health needs. Identify changes in health status and communicate concerns to appropriate providers and team members. Provide direct nursing interventions and support as needed. Promote wellness, preventive care, and healthy living practices. Monitor ongoing medical conditions and treatment plans.

Medication Management Administer medications and treatments as prescribed. Maintain accurate medication administration records. Monitor effectiveness of medications and report concerns. Educate residents about medications and health conditions. Ensure medication practices follow agency policies and regulatory requirements.

Medical Coordination Schedule and coordinate medical appointments and follow-up care. Communicate with physicians, specialists, pharmacies, and other providers. Assist with implementing medical recommendations and treatment plans. Advocate for residents during medical appointments and healthcare interactions. Ensure continuity of care among all providers and support staff.

Staff Training & Support Provide health-related instruction and support to residential staff. Train staff on individual healthcare needs and procedures. Reinforce safe medication and health-related practices. Serve as a nursing resource for program staff. Documentation & Compliance Maintain accurate and timely nursing documentation.

Ensure medical records remain current and complete. Protect resident confidentiality and privacy. Ensure compliance with agency policies, state regulations, and healthcare standards. Participate in required meetings, trainings, and quality improvement activities. Physical Requirements Ability to sit, stand, walk, bend, stoop, and reach throughout the workday. Ability to assist with resident support when necessary.
