You’ll work closely with patients, caregivers, and their care teams to improve mobility, strength, functional independence, and quality of life. Your responsibilities

Providing skilled physical therapy interventions under the direction of a Physical Therapist Providing transfer training and instruction in safe mobility techniques Educating patients and caregivers on exercises, mobility strategies, and safety techniques Monitoring patient progress and reporting changes to the supervising Physical Therapist Collaborating with Physical Therapists, nurses, occupational therapists, speech language pathologists, physicians, and other members of the care team Completing accurate and timely electronic documentation Home care offers a unique opportunity to get to know your patients and make a meaningful difference in their daily lives.

Requirements
