Student loan repayment options Pet Insurance Qualifications: High School diploma or equivilent Three years of responsible full time clerical experience Excellent customer service skills Ability to follow complex oral and written instructions Duties and Responsbilities: Assist individuals served by addressing inquiries, resolving issues, and fulfilling requests across diverse communication channels.

Responsible for answering and triaging internal and external telephone calls. Conducts initial screening of individuals serviced and other patrons to determine the nature of requests or problems. Pulling, filing, auditing, and maintaining charts for the teams assigned. Closing team charts Co-facilitating team meeting minutes including huddles regarding team-based care, integrated health and other population health.

Reviewing and sharing data reports, outcomes of team services. Assiting with coordination of care for individuals served on the team.
