As a Registered Dietitian at Humboldt House, you will be responsible for:

• Conducting nutritional assessments and developing individualized nutrition plans
• Providing medical nutrition therapy to residents with various dietary needs
• Collaborating with our interdisciplinary team to ensure seamless care coordination
• Educating residents and families on healthy eating habits and disease management
• Maintaining accurate records and documentation of resident care Responsibilities As a Registered Dietitian at Humboldt House, your responsibilities will include:
• Conducting comprehensive nutritional assessments and developing individualized care plans
• Implementing and monitoring resident nutrition plans, making adjustments as needed
