Salary adjustments are determined by program eligibility, advancement level, and organizational guidelines. Conduct comprehensive clinical assessments to evaluate patients' physical, mental, and emotional health. Develop and implement individualized care plans based on assessment findings and patient needs. Coordinate multidisciplinary care by facilitating collaboration among healthcare professionals, including physicians, nurses, social workers, and therapists.
