Case Manager II - Renewing Hope Program

The Renewing Hope Case Manager II provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual client needs. Responsible for conducting assessments for homeless clients to create a client centered individual service plan which identifies supportive services needed to transition to an independent life.

Additionally, responsible for the necessary follow up with local agencies to assure needed services are initiated and continue as planned.
