Outreach Clinical Coordinator
Job description
Provide community-based, low-barrier support to individuals experiencing mental health and/or substance use challenges and/or unmet social service needs. Focusing on mitigating and preventing crisis events and reducing reliance on emergency services, when possible, through proactive engagement, coordination, and referral to appropriate resources.
