Job description
Responsible for the implementation of quality nutritional care including patient interviews, assessment of nutritional status, intervention, and monitoring in the acute hospital and long-term care. Outpatient services areas include: Cancer Center, Diabetes Clinic, Home Health and Hospice, and Care Coordination. Focus areas include, but are not limited to, nutrition support (enteral and parental nutrition), diabetes, cardiovascular disease, gastrointestinal disease, oncology, dysphagia, and food allergies.
Interdisciplinary communication, discharge care planning, and referral to community outreach programs will also be provided as appropriate. The RDN will also serves as a resource in the development of policies and procedures related to nutritional care.
