Community Care, Inc. is looking for a compassionate and dedicated RN to provide home health care to our clients in the Westmoreland and Fayette County area. 🩺 What You'll Do

• Conduct skilled nursing visitsin patients' homes**(assessment, reassessment, medication administration, IV therapy, and treatments)**

• Develop and implement individualized care plans

• Educate patients and caregivers on managing conditions and medications

• Collaborate with a multidisciplinary team to ensure continuity of care

• Document visits and patient progress accurately and timely

✅ What You'll Need

• Current Registered Nurse (RN) license in the state of Pennsylvania.

• Graduate of an accredited nursing program

• Minimum 1 year of clinical nursing experience (Home Health experience preferred)

• CPR certification (required)

• Reliable transportation and a valid driver's license

• A caring heart, strong independence, and a passion for pediatric care
