What You’ll Do As a Home Health Physical Therapist, you will: Perform and document comprehensive initial evaluations and develop individualized treatment plans Assess patient needs and identify functional limitations prior to intervention Design and implement rehabilitative programs to improve mobility, strength, balance, and pain management Evaluate patient progress and modify treatment plans to maximize outcomes Educate patients and caregivers on exercises, mobility strategies, and safety techniques Maintain accurate and timely clinical documentation Collaborate with physicians, nurses, and interdisciplinary care teams Follow infection control and universal precaution standards during patient care Support agency philosophy, mission, and quality care standards Qualifications Graduate of an accredited Physical Therapy program Current and active Physical Therapist license Minimum 1 year of home health experience preferred CPR Certification required Valid driver’s license and reliable transportation Current auto insurance Home.

Care Home. Base experience preferred Physical Requirements Ability to stand, walk, sit, bend, squat, kneel, and climb for extended periods Ability to lift, carry, push, or pull up to 100 lbs.
