We’re seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our New Perspectives team. This is an opportunity to use your nursing skills in a behavioral health setting where your work directly supports individuals experiencing mental health challenges and crises. You’ll provide nursing care within our residential program while also supporting mobile crisis services in the community.

Our residential services provide supervised mental health stabilization as an alternative to psychiatric hospitalization. New Perspectives also provides assessment, crisis intervention counseling, crisis stabilization, referral and linkage to other services, and assessment of the need for emergency hospitalization in individuals' homes and other community locations.

You’ll assess health needs, provide medication support, coordinate with psychiatric and program staff, maintain accurate nursing documentation, and participate in mobile crisis interventions.
Your Responsibilities
Nursing Assessment & Participant Care Complete nursing assessments for individuals within required timeframes following admission to the residence. Assess participants' physical and emotional health needs. Provide nursing interventions based on identified health concerns. Monitor participant health and recognize changes requiring additional attention.
Provide face-to-face and telephone support to individuals receiving services. Communicate significant health concerns to the Nurse Manager, Program Director, and other appropriate team members. Medication Management & Education Maintain accurate nursing documentation and medication records. Assist individuals in learning about their medications and treatment.
Ensure medications are taken as ordered by the physician. Maintain accurate medication administration documentation in the electronic MAR. Refill non-controlled medications in consultation with the New Perspectives psychiatrist. Monitor participants for medication-related concerns and communicate appropriately with clinical providers.
Provide health and medication education that supports recovery and wellness. Psychiatric & Behavioral Health Support Facilitate residential telepsychiatry appointments with the psychiatrist using virtual systems. Work closely with psychiatric providers and behavioral health professionals to coordinate participant care.
Help ensure participant health information is communicated accurately across the treatment team. Support a collaborative approach to physical and behavioral healthcare. Help individuals understand health conditions, treatments, and self-care strategies. Mobile Crisis Services Participate in medical mobile and mobile crisis interventions.
Provide nursing support during community-based crisis responses. Assist with identifying health-related concerns that may be contributing to an individual's crisis. Work collaboratively with crisis professionals to support stabilization and determine appropriate next steps. Maintain professionalism, sound judgment, and a person-centered approach during crisis situations.
Documentation & Team Collaboration Maintain accurate and complete electronic health records. Document health concerns, symptoms, interventions, medication information, and changes in participant condition. Communicate effectively with physicians, nursing staff, behavioral health professionals, and other members of the interdisciplinary team.
Maintain participant confidentiality. Participate in required meetings, trainings, and program activities. Complete additional duties as assigned. 00 to $25.00/hour New Perspectives Position includes both residential and community-based services Schedule flexibility may be required based on participant and program needs. Flexibility may include evenings, weekends, and work at different locations . Physical Requirements Ability to stand, sit, and walk throughout the workday.
Ability to climb stairs, reach, bend, kneel, stoop, crouch, push, and pull as required. Ability to perform the physical responsibilities associated with residential and community-based nursing services. Ability to occasionally exert up to 50 pounds of force , frequently exert up to 20 pounds, and regularly exert up to 10 pounds to move objects.
