Case Manager I

Manage up to 15 cases Conduct intake, assessments, and initial service planning Provide referrals and support for housing placement Enter data in HMIS/SQUARES and ensure documentation compliance Conduct outreach in low-risk communities such as Summerlin, Centennial Hills, and Henderson Provide direct case management services to homeless and at-risk veterans and their families. Screen veterans’ eligibility for the program and facilitate the intake process. Coordinate care with VA personnel and others on behalf of participants for eligibility documentation, mental and physical health services.
