All-at-Home Health Care is seeking a responsible, detail-oriented Registered Nurse (RN) to join our Group Adult Foster Care (GAFC) and Adult Foster Care (AFC) Program .

GAFC/AFC Registered Nurse

• Home Health/Hospice for some flexibility in working hours. Servicing patients in all the Fall River area. You must have a car or personal transportation to make visits during 5-10 hours a day . The benefits you will get: $45-$48 pay per visit TRAVEL REIMBURSEMENT EXCEPTIONAL PAID TRAINING HOLIDAY PAY PAID SICK TIME FLEXIBLE SCHEDULE MEANINGFUL WORK EVERY DAY Responsibilities: As the Per-diem GAFC/AFC Registered Nurse , you will provide nursing oversight services that are individualized to meet the needs of each member in accordance with the member's GAFC/AFC plan of care. You will be responsible for a diverse range of duties including: Complete or coordinate all applicable clinical assessments and clinical care. Develop and review on an ongoing basis each member's GAFC/AFC plan of care. Review the PCP summary form. Review GAFC/AFC direct care aide logs at a minimum of every 30 days. Conduct a nursing progress note corresponding with each on-site visit or encounter. Report changes in the health status of any member to the member's PCP. Submit semi-annual health status reports to the member's PCP. Plan for and implementing discharges and transition from the GAFC/AFC program. Provide ongoing training to GAFC/AFC direct care aides on health and aging. Act as Case Manager when assigned by Clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload.

Requirements

Current and Valid RN MA license CPR Certificate Clear CORI background check Valid Driver's License and reliable transportation Willing to travel to all the Fall River area 1 year experience in home care is preferred What makes us unique?
