- Social Services, Connect Veterans to Support. Strengthen Hope. Help Build a Path Forward. Reduce the 22 per Day. At the Center for Veterans Issues, Inc. (CVI), every connection is an opportunity to help a Veteran move toward greater stability, safety, and well-being. As a Case Manager with our Staff Sergeant Parker Gordon Fox Suicide Prevention Program, you will help eligible Veterans, Service members, and qualifying family members access resources, overcome barriers, and build meaningful support networks. Through compassionate engagement, individualized planning, and dependable follow-through, your work will support a mission dedicated to preventing Veteran suicide. This primarily nonclinical role calls for someone who combines empathy with sound judgment, strong organization, and a commitment to turning referrals into real connections. Why Join CVI? Your work can help Veterans feel heard, access essential resources, and build connections that support lasting stability. You will contribute to a mission grounded in dignity and guided by CVI’s core values: Professionalism, Respect, Integrity, Dedication, and Excellence. If you bring compassion, initiative, and dependable follow-through to serving Veterans and their families, we would love to meet you. What You’ll Do Reach and Engage Veterans Conduct outreach, particularly to individuals unaffiliated with VA and those in rural or underserved communities. Explain services, participant rights, confidentiality, and VA enrollment opportunities. Use trauma-informed, culturally responsive engagement and document outreach, enrollment decisions, and barriers. Support Eligibility and Screening Verify military-status eligibility and document required suicide-risk screening, degree of risk, VA affiliation, notices, and baseline-screening status before services begin. Use approved screening tools within your training and authorization, identify participant strengths and needs, and immediately escalate urgent safety or clinical concerns. Build Individualized Plans and Coordinate Support Develop and update service plans that reflect each participant’s priorities, risks, strengths, and protective factors. Coordinate approved resources addressing housing, healthcare, employment, benefits, transportation, basic needs, and family or recovery supports. Assist with safety planning and crisis coordination when trained and authorized, while maintaining nonclinical boundaries and following field-safety procedures. Turn Referrals into Connections Initiate required VA and community referrals and provide warm handoffs when practicable. Help participants navigate scheduling, transportation, and authorized information sharing. Collaborate with VA facilities, County and Tribal Veteran Service Offices, the Wisconsin Veterans Outreach and Recovery Program, and community providers. Follow referrals to documented outcomes and support transition planning at exit. Maintain Reliable Records and Follow-Through Keep timely, accurate, confidential, and audit-ready records of screenings, plans, contacts, referrals, assistance, progress, and exit activities. Manage your assigned caseload, meet participant-contact expectations, and document missed contacts and outreach attempts. Participate in supervision, case staffing, training, and quality reviews; promptly communicate safety, workload, or compliance concerns. What Success Looks Like Timely engagement: Meet assigned caseload, enrollment, and participant-contact expectations while maintaining service quality and professional boundaries Complete enrollment records: Document required eligibility, risk, VA affiliation, notices, and baseline-screening status for 100% of enrolled participants before grant-funded services begin Responsive safety practices: Escalate and document 100% of urgent safety concerns according to policy; complete and update 100% of service plans within established timelines Coordinated referrals: Offer, initiate, and document 100% of required VA referrals; provide warm handoffs when practicable and document refusals or unsuccessful connections Reliable documentation: Complete case records within required timelines and quality standards, including progress, missed contacts, and outreach attempts Thoughtful transitions: Document required exit-screening, transition-planning, final-referral, and satisfaction-survey status for 100% of exiting participants Meaningful program outcomes: Contribute to program targets of at least 40% unaffiliated with VA at intake, 75% receiving VA referrals, and 85% of program completers improving in at least one required outcome domain Continued development: Complete required supervision, meetings, and training on time unless specifically excused We are an EEOC/AA/ADA employer; Veterans are strongly encouraged to apply.
