Summary of Responsibilities: Make accurate and ongoing data collection and charting of the resident’s condition and status. Communicate with the physicians regarding resident’s needs and changes in their conditions.

Collaborate with others to ensure the residents’ well-being. Assist resident’s and family members with concern and empathy. Respect their confidentiality and privacy and communicate with them in a courteous and respectful manner. Administer medications to residents in an accurate, timely manner. Monitor and document medication for residents for effectiveness of drugs and occurrence of adverse effects.

Assists residents with providing hands on care as needed. Support, participate in developing and following residents individualized plan of care. Qualifications:
