This hybrid position will be based out of either our Jasper, IN or Fordsville, KY health center, depending on candidate location. You'll support multiple Kimball health centers through a structured weekly schedule that combines onsite patient care with one remote workday each week. Travel between locations is planned in advance and allows you to build trusted relationships with patients and care teams across the region.
You'll provide evidence-based counseling and care coordination for patients experiencing anxiety, depression, trauma, stress, substance use, relationship concerns, parenting challenges, and other behavioral health needs.
What You'll Do
Provide evidence-based individual and family counseling Partner with primary care providers to deliver coordinated, whole-person care Develop treatment plans and maintain timely clinical documentation Coordinate referrals and connect patients with specialty services and community resources when appropriate Support wellness initiatives, educational programming, and patient engagement Build collaborative relationships with patients, providers, clinic staff, and employer partners Travel between assigned health centers as part of a structured schedule What You'll Bring Master's degree in Social Work (MSW) from a CSWE-accredited program or a related behavioral health field Current, active professional license in good standing within the applicable state(s) Minimum one year of recent experience providing evidence-based individual counseling Experience with electronic medical records and clinical documentation Ability to travel between assigned health centers on a structured weekly schedule Multi-state licensure or willingness to obtain additional state licensure as needed Experience providing mental health services in a primary care setting preferred Location & Travel This position is based out of either our Jasper, Indiana or Fordsville, Kentucky health center, depending on candidate location, and supports multiple Kimball health centers throughout the region.
The role includes one remote workday each week , with onsite care provided at assigned health centers following a structured, predictable schedule.
