Job description
The Medical Case Worker II supports Medi-Cal–eligible adults involved in the justice system as part of California’s Medicaid 1115 JI Reentry Initiative. This role supports individuals transitioning from incarceration in state prisons, county jails, back into the community. The Medical Case Worker II works with clients during incarceration and post-release to ensure continuity of Medi-Cal coverage, access to comprehensive health services, medications, and supportive resources, including linkages to Enhanced Care Management (ECM).
The position plays a critical role in improving health outcomes, supporting successful reentry, and reducing recidivism through coordinated, client-centered care.
