Family Team Conference Facilitator (FFT-CW)

The Family Team Conferencing (FTC) Facilitator will report to the Program Director and will be responsible for scheduling, coordinating, facilitating, and tracking all various family team conferences for the program. The FTC facilitator will also gather reports on conference occurrence for purposes of QA compliance. The incumbent will understand and promote the organization’s Mission, Vision, and Core Values to ensure alignment with the functions of the position.
