This includes conducting patient admissions, developing care plans, and coordinating care with the interdisciplinary team.

Key Responsibilities

• Obtain medical history and assess patient condition and needs
• Conduct physical assessments, including vitals and mobility evaluations
• Educate patients and families on care plans and home health services
• Deliver skilled nursing care while maintaining HIPAA compliance
• Document and submit accurate visit records promptly
• Develop and update individualized care plans as needed
• Coordinate with LPNs, Home Health Aides, and Personal Care Attendants
• Facilitate referrals for additional services such as therapy or medical social work
• Evaluate the effectiveness of nursing services and adjust as appropriate Qualifications: Qualifications:
• Active RN license in OH
• Experience with OASIS and Well. Sky (Kinnser) strongly preferred
• Med-surg and wound care experience a plus
• Current TB test (within last 12 months) and CPR certification
• Valid driver’s license
