Behavioral Health Home Visitor
Job description
As a Behavorial Health Home Visitor in the Direct Support Services Program, you’ll make a lasting impact by empowering families and strengthening parenting skills. Through weekly visits, you’ll help build family stability and improve behavioral health outcomes for children. Working closely with the family’s behavioral health home and Child and Family Team (CFT), you’ll provide parent coaching, parenting education, skills training, and case management.
You’ll also complete intakes, assessments, individualized treatment plans, and connect families to vital community resources. This role is all about collaboration, compassion, and creating brighter futures for children and families.
