Bilingual Community Liaison / Intake Coordinator (English/Spanish)

About the role

A Peace of Mind Caregivers is growing, and we're looking for a bilingual (English/Spanish) Community Liaison / Intake Coordinator to build referral relationships across Las Vegas and welcome new clients into our home care services. Full-time | Pay: $18/hr base + performance bonuses | Location: Las Vegas, NV (field + office) What you'll do

• Visit hospital discharge planners, Medicaid HCBS case managers, Nevada Medicaid plans, primary care offices, senior centers, churches and elder law attorneys to build referral relationships
• Receive new referrals and complete the initial client intake
• Get new clients started on service within 24–48 hours
• Help keep Medicaid prior authorizations renewed on time
• Keep our Google Business Profile and care-directory listings up to date Goals for your first 90 days
• About 20 referral-source visits per week - 4–6 new clients per month Qualifications
• Fluent in English and Spanish (required)
• Valid driver's license, reliable vehicle and current auto insurance
• Strong communication, organization and follow-through Preferred
• Home care, home health or Medicaid experience
• Existing referral contacts in the Las Vegas healthcare community Pay - $18/hr base, full-time
