RN Case Manager - Psych

Department: Psychiatric Department/ Emergency Room Hours: Full-time, 40 hours per week, 8 hours per shift, day/evening coverage, rotating weekends and holidays as required. Basic Function : This position is responsible for coordination and oversight of evaluations in the Emergency Department and admissions (at HVB only), discharge planning activities, coordinating referrals and aftercare services for patients.

Qualifications

Required: Graduate of an accredited school of nursing with current Pennsylvania license. Successful completion of applicable clearances as outlined in Human Resources policy HR-106 within 90 days of commencing employment. CPR certification. Must have a minimum of 3 years current experience in an inpatient psychiatric setting.

Knowledge/ability to assess and utilize community resources. Preferred: Previous experience with psychiatric case management. ANA certification as a psychiatric nurse.
