Join our Primary Care team with Saratoga Hospital Medical Group Compensation $122,000 - $162,000 We are seeking a Nurse Practitioner or Physician Assistant to join our growing Primary Care team. Our practices bring together board-certified physicians and advanced practice providers experienced in both adult and pediatric medicine, with a shared commitment to providing compassionate, patient-centered care.

As part of our team, you#ll work collaboratively with primary care providers and specialists to deliver coordinated, high-quality care while enjoying a meaningful level of autonomy in managing your own patients. We are currently growing our team at the following locations: Primary Care

• Saratoga: 381 Church St, Saratoga Springs, NY# Call is 1:15, shared with providers.# As a Nurse Practitioner or Physician Assistant, you will evaluate, diagnose, and manage patients while working closely with physicians, APPs, and other members of the care team.

Responsibilities include

Obtain, document, and update patient medical histories Perform physical examinations and conduct preventive screening procedures based on age and history. Identify patient health risks and clinical needs Develop differential diagnoses and individualized plans of care Order and review diagnostic testing Prescribe and manage appropriate pharmacologic therapies Evaluate patient outcomes and adjust treatment plans as needed Coordinate referrals to specialists and community resources Facilitate admission or discharge to the appropriate level of care when indicated Collaborate in team meetings and interdisciplinary care discussions Participate in quality improvement initiatives and the development of standards of care Maintain current clinical knowledge and professional competencies Qualifications: Current New York State license to practice as a Nurse Practitioner or Physician Assistant Current required professional credentials and certifications, including applicable licensure, BLS/ACLS, infection control, and DEA registration Commitment to collaborative, compassionate, patient-centered care Experience with Epic is preferred, but not required About Saratoga Hospital Medical Group Saratoga Hospital Medical Group, a multispecialty group of 450#providers including physicians and advanced practice providers, contributes significantly to the hospital#s continued growth and reputation for excellence.
