Nurse Practitioner Specialty - Home Visit Team

Southcentral Foundation (SCF) serves Alaska Native and American Indian people within the Anchorage service area. The Home Visit Provider delivers comprehensive, customer-owner-centered medical care to individuals in their homes, with a focus on supporting those with complex medical needs and barriers to accessing traditional clinic-based care.

This role requires an understanding of the cultures, values, and unique needs of the population served, while working collaboratively with an interdisciplinary team to promote continuity of care, independence, and overall wellness. The Home Visit Provider supports SCF’s vision of a Native Community that enjoys physical, mental, emotional, and spiritual wellness.
