Job description
Are you a passionate Physiotherapist looking for a rewarding role that offers flexibility, autonomy, and the opportunity to help older Australians remain independent in their own homes? We are seeking a Part-Time Community Physiotherapist to join our growing team in Adelaide, supporting clients accessing services through the Support at Home (SAH) and Home Care Package (HCP) programs.
You are an experienced and compassionate community Physiotherapist who enjoys building strong relationships and delivering person-centred care. Essential Requirements Current AHPRA registration as a Physiotherapist. Current Australian Driver's Licence and reliable vehicle. National Police Check (or willingness to obtain).
Current First Aid and CPR certification Excellent communication and time management skills. Ability to work independently while being supported by a collaborative team. Community aged care experience is highly regarded but not essential. Working in the community, you'll provide evidence-based physiotherapy to improve mobility, reduce pain, prevent falls, and support clients to live independently at home.
You'll work with clients, families, and other health professionals to deliver personalised care that promotes healthy ageing.
Key Responsibilities
Conduct comprehensive physiotherapy assessments in client's homes and recommend mobility aids and equipment where appropriate Develop and implement individualised treatment and exercise programs. Provide falls prevention, mobility, balance and strength interventions. Educate clients, families and carers to maximise independence and safety.
Complete clinical documentation in line with funding requirements and organisational Policies Liaise with GPs, allied health professionals and support coordinators as part of a multidisciplinary approach.
