Hospice Intake Nurse

Coordinates intake for the Calvary Hospital Hospice/Home Care. Assists in the explanation of the Agency’s eligibility criteria and reimbursement sources to patients, families and community. May attend Discharge Planning Conferences at local hospitals to plan for Hospice/ Home Care Services. Serves as educational liaison to hospital(s) concerning the range of Hospice and Home Health Services.

Communicates, interprets and promotes Agencies’ philosophies, policies and procedures to discharge planning, nursing, medical and other hospital staff, as well as community agencies, patients and families. Assists in the development of policies and procedures for effective intake of cases and communication of intake information.

Collects third party payor information and communicates with billing office to ensure acquisition of correct billing information. Communicates regularly with management of Hospice and Home Care to assure a smooth flow of information.

Qualifications

Current New York State Registered Nurse License/Permit.

Education

Associate Degree BSN from an accredited school is preferred. Salary: $113,576 - $140,076 annually
