You will serve as an important member of the interdisciplinary care team and help patients remain safely and comfortably in their homes.

Responsibilities

Conduct comprehensive nursing assessments during home visits Develop, implement, and update individualized patient care plans Provide skilled nursing care in accordance with physician orders and applicable regulations Monitor patient conditions and identify changes requiring intervention Coordinate care with physicians, therapists, aides, social workers, and other members of the healthcare team Educate patients and caregivers regarding medications, disease management, treatment plans, and preventative care Complete accurate and timely clinical documentation Communicate effectively with patients, families, physicians, and the interdisciplinary team Manage an assigned caseload while maintaining high standards of patient care Help patients and families navigate their healthcare needs and achieve their goals Qualifications Current, unrestricted RN license in the State of Connecticut Previous home health experience strongly preferred Strong clinical assessment and critical-thinking skills Excellent communication and organizational abilities Ability to work independently while collaborating effectively with a team Reliable transportation and willingness to travel to patient homes throughout the Stratford area Commitment to providing compassionate, patient-centered care The Culture Matters Here We believe nurses do their best work when they feel supported, respected, and appreciated.
