Murfreesboro Primary Care- LPN/RN Posting Title: Licensed Practical Nurse (LPN) Reports To: Nurse Manager Direct Reports: None Classification/Category: Clinical Level I Workgroup(s) : N/A Work Status : On Site Salary Range: Stipend(s): None Status: Non-Exempt Summary of Position The Licensed Practical Nurse plays a critical role in the delivery of quality medical care with in the center.

This position requires a person with strong clinical skills along with assessment and communication skills. The Licensed Practical Nurse provides care as described by the established policies and procedures of RCCHC. This position will provide clinical care in collaboration with other partnerships to achieve positive patient outcomes.

Minimum Qualifications

Must demonstrate competencies in nursing practice Works effectively with others and is able to organize workload, as well as assume responsibility Must be able to work independently, exercise appropriate decision making skills, and function effectively on a team Must be able to speak and communicate clearly and effectively Education : Current license as a LPN in state of North Carolina required Experience : Work experience in a medical practice preferred Certification(s)/Licensure : BLS certification required Knowledge: Knowledge and thorough understanding of Federally Qualified Health Centers (FQHC’s) in regard to the Federal 330 Public Health Service Act Knowledge of thorough understanding of the 340B program Knowledge of computer systems and applications.

Skills: Skill in exercising a high degree of initiative, judgment, and discretion. Skill in analyzing situations accurately and taking effective action. Skill in establishing and maintaining effective working relationships with the medical and administrative staff, patients and the community. Skill in planning, organizing, and prioritizing work; delegating and achieving goals and objectives.

Skill in exercising judgment and discretion in developing, interpreting, and implementing departmental policies and procedures. Skill in identifying and resolving problems. Skill in developing comprehensive reports. Abilities: Ability to plan, organize, and integrate priorities and deadlines. Ability to identify, analyze, and interpret complex data and, resolve operational problems.

Ability to evaluate and make recommendations for continuous quality improvement. Ability to evaluate and make recommendations for continuous risk assessment. Ability to anticipate and react calmly in emergency situations. Ability to communicate clearly and effectively orally and in writing. Ability to competently use Microsoft Office, including Word, PowerPoint, Excel, and appropriate organizational software.

Essential Functions

Perform independent nursing functions using RCCHC established procedures, policies, and guidelines. Administer medications/immunizations as ordered by RCCHC providers, documenting accurately and observing patients for reactions Answer and return patient phone calls and other inquires to ensure accurate and timely communication is facilitated between all members of the care team Complete paperwork for assigned providers as per RCCHC guidelines Acts as a secondary point of contact for urgent or “walk-in” patients or patients telephoning in with urgent care complaints Establish reason for patient’s visit, completing the rooming process per established guidelines and documenting accurate data in the EHR Assist patients in the preparation for the provider’s examination, including undressing, assisting to the table, and dressing Accurately review and record medications and /or procedures as directed by the provider Accurately execute technical procedure when ordered such as EKG’s, drawing blood, obtaining and preparing laboratory specimens Maintain proper storage and handling of equipment, supplies, and medications Maintain a clean and orderly clinical working atmosphere; adequately prepare exam rooms and properly equip laboratory Work with patients and family in giving accurate, understandable instructions in diet, special care procedures, medication, patient education Participate as an active member of the RCCHC team Maintain compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation Other job duties may be required that are not listed above.

Prioritize nursing actions including reflecting patient needs, acuity, and desired outcomes  Implement physician orders accurately within appropriate time frame. Provide patient care based on scientific principles/nursing theory and evidence based practice. Reviews data collection and assesses patient complaint and request for service.

Evaluates provider schedule and determines appointment vacancies. Collaborate with nurses (RNs and LPNs) using critical thinking in clinical problem-solving and decision-making processes relative to scope of practice, knowledge, competency, and experience Perform independent nursing functions using RCCHC established procedures, policies, and guidelines.

Provide nursing intake, triage, assessment and education to patients via telephone, face to face interactions, and secure messaging. Delegates tasks to lab, nursing, and clerical staff judiciously. Collaborates with all staff members to enhance positive patient outcomes.

Makes empowered decisions in response to patient needs. Communicates effectively, presenting positive and professional image in all interactions. Assists with precepting students and/or new employees Adheres to organizational and departmental policies and procedures. Maintains a clean and safe environment. Knowledgeable of and enforces RCCHC safety policies and workplace safety rules.
