Schedule : Monday
- Friday | 8:00am - 4:30pm As a Social Worker, you will provide psychosocial support to patients and families, ensuring they have the resources, guidance, and emotional care needed during end-of-life care.
Your role
will be essential in assessing needs, coordinating support services, and guiding bereavement care, fostering an environment of dignity, comfort, and peace for patients and their loved ones. As a Social Worker, you will: Provide Psychosocial Support
- Conduct initial and ongoing accurate and thorough psychosocial assessments at each visit, including evaluating family dynamics, caregiver abilities, and emotional needs while in the patient’s home. Active Member of IDG
- Communicate and collaborate continuously with the interdisciplinary team, including RN, spiritual care and other members of the patients care team. Initiate and participate in care conferences to discuss patient needs and goals of care. Complete IDG notes and updates in a timely manner. Offer Bereavement & Emotional Support
- Ability to perform a bereavement risk assessment for patients’ family or care giver and carry out risk assessment findings as applicable. Collaborates with the Bereavement Team to ensure that all bereavement support identified is provided. Advocate for Patients & Families
- Identify and connect families to community resources, financial support, and emotional wellness programs to ease stress during care. Ensure Compliance & Documentation
- Maintain accurate, up-to-date records in accordance with hospice policies, regulations, and best practices. Other Duties as Assigned
- The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive. This job also requires: Bachelor’s degree in Social Work or related field required. Minnesota Social Work License preferred; if licensed, must maintain continuing education requirements. Experience in hospice, healthcare, or geriatric care preferred. Strong assessment, problem solving, and crisis intervention skills. Ability to communicate with empathy and professionalism with patients, families, and care teams. Ability to pass state mandated background check. Physical capability to perform essential job functions. Ability to read, write, and speak English to ensure effective communication with staff, clients, and families. Additional Details: Employment Type: Hourly, non-exempt Department: Social Services Leadership Received: Family Services Manager Division: Hospice Travel Requirements: Travel to patients homes within designated regions. This role does not include supervisory responsibilities
