Key Responsibilities

• Direct and supervise social service programs to ensure quality and compliance with organizational standards.
• Evaluate the social needs of each resident upon admission and develop comprehensive written care plans.
• Review and revise care plans at least quarterly or as needed to adapt to residents' changing needs.
• Refer residents and their families to appropriate community resources and support services.
• Counsel residents, family members, and support staff through assessment and care planning processes.
• Maintain up-to-date knowledge of community agencies, social services, and local resources to support residents effectively.
• Attend in-service training, online courses, and other educational opportunities to maintain professional credentials and stay current with best practices. Skills and Qualifications:
• Bachelor’s degree in Social Work or related field.
• Knowledge of sociology, special education, rehabilitative counseling, and psychology.
• Proven ability to counsel residents and support staff effectively.
• Strong understanding of community resources, social agencies, and local services.
• Excellent communication, interpersonal, and organizational skills.
• Ability to assess individual needs and develop tailored care plans.
