HomeCentris Healthcare, LLC has a portfolio of companies that offer a full continuum of home-based healthcare services designed to allow our patients to remain at home, with their family and loved ones, rather than a more institutional senior care residence. Our Medicare-certified home health agency, HomeCentris Home Health, offers skilled nursing and therapy services that can start immediately after a hospital discharge. Our Personal Home Care subsidiary can help a client transition to a daily care plan of personal care services for the Activities of Daily Living (“ADL”). Finally, our HomeCentris Community Care subsidiary can bring primary care right to our patient’s residence. All of our services are designed to allow our clients to remain at home, where they want to be.
