Home Health Aide 8am-4:30pm

Schedule : 40 hours per week | Monday – Friday with Holiday Rotation | 8:00 am – 4:30 pm Department: Home Care | LifeCircles PACE | Holland MI What You'll Do In This Role: LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) team.

In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence. Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.

Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping Support Mealtime– Help participants enjoy their meals and stay hydrated for good health. Keep Things Moving – Support mobility, including turning, positioning, and safe transfers. Perform Vital Checks – Measure and record vital signs, weight, and intake/output.

Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials. Provide Responsive Support – Respond with kindness and compassion. Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP. Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Reliable transportation and valid driver’s license, required. High school diploma or GED, required. Excellent communication and observation skills.
