Bilingual Community Health Worker Per Diem (Rockland County)

LOCATION: ROCKLAND COUNTY INCLUDING New City (10956), Nyack (10960), Orangeburg (10962)

PAY RATE: $25 Per hour

JOB TYPE: Per Diem

POSITION SUMMARY:

The Community Health Worker is responsible for visiting patients' houses to conduct nonmedical screenings as required by the Insurance Companies. The Community Health Worker helps to bridge the gaps between the community and healthcare services.

RESPONSIBLITIES:

• Travel to patients' homes as determined by need and schedule
• Conducting home visits to complete non medial surveys both initially and in six months per each case
• Ensure patient is residing in a safe environment
• Responsible for implementing patients care activities

PHYSICAL REQUIRMENTS:

• Must be able to carry, lift, push or pull at least 20lbs

REQUIREMENTS:

MUST BE BILINGUAL

• Must have own reliable means of transportation to drive throughout the areas listed
• Highly organized
