Position Summary

As a Nurse Practitioner (NP) with Impact Health PC in Augusta, ME you will provide comprehensive Medical/Primary Care to patients within skilled nursing facilities. You will assess and manage acute and chronic medical conditions, develop individualized treatment plans, provide ongoing patient follow-up, and collaborate with physicians, facility staff, specialists, and interdisciplinary care teams.

Key Responsibilities

Provide comprehensive Medical/Primary Care to residents within skilled nursing facilities. Assess, diagnose, and manage acute and chronic medical conditions. Perform routine patient evaluations and ongoing follow-up care. Develop individualized, evidence-based treatment plans. Order and review appropriate medications, laboratory tests, imaging, and other diagnostic studies.

Complete timely and accurate clinical documentation and place appropriate orders. Collaborate with physicians, nursing staff, specialists, and interdisciplinary care teams. Educate patients and families regarding medical conditions and treatment plans. Maintain compliance with facility, state, and federal regulations.

Minimum 2 years of post-certification clinical experience preferred. Experience in skilled nursing facilities, primary care, hospitalist medicine, emergency departments, or intensive care preferred. Strong clinical assessment, communication, and organizational skills. Comfortable managing acute and chronic medical conditions in an older adult population.

Ability to work independently within scope of practice and collaboratively with an interdisciplinary care team. Proficiency with EHRs and clinical technology. #HP123
