The RN/LPN is responsible for providing high-quality nursing care to residents in accordance with physician orders, individualized care plans, facility policies, and applicable state and federal regulations. The ideal candidate will demonstrate strong clinical skills, excellent communication, and a commitment to promoting the health, safety, dignity, and well-being of our residents. Essential Job Duties & Responsibilities Provide direct nursing care to residents in accordance with established standards of nursing practice. Administer medications and treatments as prescribed by physicians and authorized providers. Monitor, assess, and document residents' physical, mental, and emotional conditions. Identify and promptly report changes in resident conditions to the appropriate physician, supervisor, and responsible parties. Participate in developing, implementing, and updating individualized resident care plans in collaboration with the interdisciplinary team. Perform wound care, medication administration, treatments, and other nursing procedures within the scope of licensure. Supervise and provide guidance to Certified Nursing Assistants (CNAs) and other assigned nursing staff within the scope of the nurse's role. Ensure accurate and timely documentation in the electronic medical record system, including Point. Click. Care (PCC). Assist with resident admissions, discharges, transfers, and ongoing assessments. Follow infection prevention and control practices, safety procedures, and emergency protocols. Communicate effectively with residents, families, physicians, and other healthcare professionals. Maintain compliance with HIPAA, resident rights, and facility policies. Respond promptly and appropriately to resident emergencies and changes in condition. Participate in staff meetings, training, quality improvement initiatives, and continuing education. Promote a positive, respectful, and supportive environment for residents, families, and coworkers. Perform additional nursing duties as assigned by the Director of Nursing or Administrator, consistent with licensure.

Qualifications

Required Qualifications Must possess a current, active, and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois. Must be a graduate of an approved nursing program. Must maintain current CPR/BLS certification as required by facility policy. Must demonstrate knowledge of nursing practices, medication administration, infection control, and resident safety.

Must possess strong organizational, communication, and critical-thinking skills. Must be able to work independently and collaboratively as part of a healthcare team. Must be able to accurately document resident care and maintain confidential medical records. Must demonstrate compassion, patience, professionalism, and respect when caring for elderly and vulnerable residents.

Must meet applicable background check and employment eligibility requirements. Must be able to perform the essential functions of the position, with or without reasonable accommodation.

Preferred Qualifications

Previous nursing experience in a skilled nursing facility, long-term care, rehabilitation center, or similar healthcare setting preferred. Experience with Point. Click. Care (PCC) or other electronic medical record systems preferred. Experience supervising nursing assistants and coordinating resident care preferred. Knowledge of Medicare, Medicaid, and CMS nursing facility regulations is beneficial.
