Position Description: We’re on the hunt for a Hospice Liaison who knows how to build genuine relationships with referral sources, communicate compassionately with families, and never backs down from a good parking lot conversation at an assisted living facility.

Position Requirements: Explaining hospice with empathy, warmth, and not sounding like a textbook. Juggling facility visits, calls, family meetings, and coffee runs like a pro. Being a familiar, friendly face to our referral partners. Bonus points if you: Can find your way around a SNF blindfolded. Know that hospice isn’t giving up, it’s showing up differently.
