Social Worker - Long-Term Care

As a Blakeford Social Worker, you will: Counsel residents and their families on emotional, environmental or physical observations and interventions. Conduct regular face-to-face visits with assigned residents Facilitate active participation in care planning Serve as resident liaison for social, health, and community services that meet individual needs throughout discharge planning activities.
