LiveWell Member Care Coordinator

Live. Well at Home is a Blakeford Senior Life program that provides a comprehensive lifetime plan to older adults who wish to live at home as they age. The Live. Well Care Coordinator serves as wellness coach, healthcare navigator, and member advocate across providers, programs, and resources that enable members to stay healthy, engaged, independent and thriving while aging in place.

This role requires regular travel throughout the middle Tennessee area to clients' homes and/or their appointments. Mileage reimbursement is available and reliable transportation is required.
